3) That may be the best you can do for now. If the institution was
registered with a URI (for example, in the Biodiversity Collections
Index - http://www.biodiversitycollectionsindex.org/static/index.html),
the URI would be a preferable ID.
6) I'm not sure what you mean by "hide it if it is not present". But
if there is bona fide name, put it in recordedBy. If there isn't, use
<dwc:recordedBy xsi:nil="true"/>
